Kelso gira ancora per la caffetteria a caccia di muffin (ha ancora la fornitura a vita) ma ora viene visto in una luce del tutto nuova dal personale, fino a quando JD e Elliot si accorgono che praticamente Kelso vive nella caffetteria e cercano di aiutarlo fino a quando Kelso pensa che loro due stiano di nuovo insieme e se ne va, JD e Elliot allora cominciano a ripensare al loro rapporto, al suo ritorno Kelso gli fa capire che se non ci si parla col cuore a nessuno importerà di ciò che si dice. Intanto alla luce del licenziamento di Maddox l'Inserviente e Jimmy "mal del tocco" sono stati riassunti, Ted è felice del suo ritorno fino a quando l'Inserviente lo costringe a fare il suo lavoro rivelandogli che lui in realtà è solo tornato all'ospedale e in realtà non é stato riassunto, sarà riassunto infatti solo a fine puntata, il Dr.Cox e Turk continuano con le loro litigate tirando dentro anche il Todd e un paziente e sarà proprio il Todd a far capire al Dr.Cox che se si fidasse un po' di Turk i loro dissidi finirebbero.
